Dr. Elaine Colby
Dr. Elaine Colby is not only a dedicated and compassionate family physician, she is passionate about reforming the quality and delivery of health care. After seven years of caring for her patients at a local medical group in Birmingham, Alabama, Dr. Colby is excited to be opening her own practice in May of 2024 - Dalton Health Direct Primary Care.
Dalton Health will be a patient-centered, membership based, Direct Primary Care practice seeking to foster strong, long-lasting relationships with their patients: Ultimately helping their patients achieve optimal well-being in all aspects of their lives.
A little about Dr. Colby’s background - After graduating from medical school, Dr. Colby completed her residency in Family Medicine at the University of Hawaii. Dr. Colby then worked in the Department of Family Medicine at Brook Army Medical Center in San Antonio, Texas from 2011 to 2015 where she received the Achievement Medal for Civilian Service in 2015.
Upon returning to Birmingham, she worked at the Birmingham VA Medical Center until joining Southview Medical Group in 2016. She values the relationships she has built with her patients during her years at Southview Medical Group and remains a trusted resource in their healthcare journey.
Dr. Colby’s enduring commitment to her patients exemplifies her compassionate and patient-centered approach to medicine.

Direct Primary Care (DPC) is a healthcare model where patients pay a monthly fee directly to their doctor for personalized, primary care services without involving insurance.
At Dalton Health DPC, this monthly membership fee covers the cost of routine visits, routine annual labs, the management of chronic conditions, acute-care visits, Telehealth calls, directly messaging your provider, and more! We also offer additional cost savings for our members on additional labs, imaging, and certain medications.
Dalton Health focuses on strong patient-doctor relationships, longer appointment times, and an increased accessibility to healthcare services without copays or deductibles.
